Updated – Using PatientChase to target patients who need an NHS Health Check

7th Oct 2021

There is now a more integrated way to import NHS Health Check data into PatientChase so it forms part of our Search Builder rather than importing it as a bespoke search.

Either way, the criteria is the same and you can choose whichever method you prefer, but this new integrated method allows you to combine such patients with certain chronic diseases, thereby creating a more combined invite.

The criteria for defining NHS Health Check patients is the same as it is in EMIS:

  • Aged 40-74
  • Not had a Health Check in the last 5 years
  • Do not have any of the following – CHD, CKD, diabetes, hypertension, AF, HF, PAD, stroke/TIA.
  • Not on statins in last 6 months
  • Qrisk <20
  • Patients who do not have familial hypercholesterolemia

The guide on integrating NHS Health Check patients into our Search Builder can be found here

The most useful and user friendly software that I have encountered in over 30 years of working with management systems.

Cheryl Barton, Data Manager – Bramblys Grange Medical Practice, Hampshire

I am so glad that we as a practice have purchased PatientChase – it has completely revamped our recall system, has been invaluable during flu recall season and more importantly shows us where the ‘holes’ are when we get to near the end of the QoF year. Many thanks!

Jackie Hogg, Clinical Data Manager – The Baffins Surgery, Portsmouth

PatientChase is an excellent product of which I think as soon as you purchase it you will ask yourself ‘how ever did I cope without it?!

Rachel Mardlin, Health Informatics Facilitator – Sheffield University Health

This product will make my QOF staff member’s job so much simpler and is a quarter of a QOF point for me

GP, Newcastle

This is exactly what we’ve been looking for

PM, Cambridge

PatientChase is extremely user friendly and the support provided by PatientChase has been excellent

Practice Administrator, Otley

By using PatientChase, what would normally have taken half a day has just taken 10 minutes!

GP QOF Lead, North Wales

We use PatientChase and find it invaluable, it saves a lot of time and helps us organise our work effectively

Sue Careswell, Leckhampton Surgery

As a practice we have relied upon PatientChase to streamline our patient recall system for several years now. I consider this an essential tool

How PatientChase helps with CQC inspections concerning patients on specific medications

New Search - PCN DES BP search now in PatientChase

Some of you will be aware that some CCGs have introduced a new BP DES requirement to be undertaken by PCNs and therefore GP surgeries. We have added this search to our Search Builder to enable practices to easily identify such patients and to optionally combine this search with any of our other searches

Important – Are you using Advanced Searches or importing other extra searches into PatientChase at the point of synchronising with EMIS? If so, please read this update.

Many practices load extra data into PatientChase such as ‘review done’ data, medication review data, weight management data etc. If you are familiar with this, then you will need to take action. This only applies if you export searches from EMIS prior to syncing PatientChase with EMIS. If you simply ‘EMIS Sync’ PatientChase with EMIS when you open it, then you do not need to take action and please continue to use PatientChase as normal.
