COVID-19 at risk searches available in PatientChase

Our medical director has put together a set of searches of at risk patients for contacting with the latest coronavirus information and guidance. These are a combination of EMIS searches and PatientChase co-morbidity searches.

A number of our practices are already using PatientChase to contact these patients.

Patients with any of the following criteria appear on the lists.

1. Hx of Splenectomy
2. Asthma and more than 10 issues of Salbutamol in the last 12 months
3. Asthma and answered yes to all 3 RCP questions in the last 2 years
4. CKD 4 or 5
5. Hba1c >75
6. Heart Failure Patients
7. Housebound Patients
8. Moderate or Severely Frail on eFi
9. Liver Disease
10.COPD with MRC >= 3
11.COPD with 4 or more course of steroids in the last year
12.Neurodegenerative Disorders (but not stroke or Tia)
13.Over 80 year old
14.Multiple Co-morbidities of AF,ASTHMA,CANCER,CHD,CKD,COPD,DM,PLD,STROKE,DEMENTIA

Coronavirus - Using PatientChase to identify and communicate with at risk groups

With the presentation situation, patients with symptoms are being encouraged to stay away from the surgery. Use this opportunity to pro-actively keep your patients up to date with information about how Covid-19 affects them and the surgery.
